DTC Submission Forms Updated - Jan 2010

The NSW TAG DTC submission template forms, for formulary applications and individual patient use applications, have been updated based on feedback from NSW clinicians and the NSW TAG membership. Click here to read more.

 

We have moved - Dec 2009

The NSW TAG office has relocated to 26 Leichhardt Street Darlinghurst. The new office is located behind the main buildings of St Vincents Public Hospital on the corner of Leichhardt and Boundary Streets.

We welcome TAG and TAGNet members and our QUM colleagues to visit our new office when you are next in the Darlinghurst area. Please note that all of our contact details, including our PO Box are the same.

Report from the Medication Safety Self Assessment - November 2008

This report provides an analysis of the aggregate data from all NSW facilities completing the MSSA between February and November 2007. The report also provides insights into where specific effort can be made across the state to improve the safety of medication use.
